  • Injury: Rotator Cuff Tear

  • Claimant sustained injury after lifting a heavy object at work. Claimant underwent surgery to repair the torn rotator cuff. Claimant underwent work hardening therapy post-operatively and was able to return to pre-injury employment.

  • Claimant was attacked by a dog at work. Claimant made a claim based on the scarring due to the dog bites.

 

  • Injury: Bilateral carpal tunnel syndrome

  • Due to Claimant’s occupation as a mechanic, the claimant was diagnosed with bilateral carpal tunnel syndrome. Claimant underwent surgical release to both wrists. Claimant was able to return to working as a mechanic with limitations on fine motor activities due to continued numbness.

  • Claimant was exposed to hazardous substances while at work. Claimant was seen at the emergency room for irritation.

 

 

  • Injury:             Spondylolisthesis at L4-5 resulting in narrowing and a herniation at L5-S1
  • Body Part: Lumbar Spine
  • Amount: $62,600.00
  • Client was involved in a motor vehicle collision while working as a tractor trailer driver. After conservative care failed, an MRI revealed degenerative spondylolisthesis at L4-5 resulting in spinal canal narrowing and a disc herniation at L5-S1.  The injured worker underwent a series of three spinal epidural injections, which did not bring relief.  The Claimant was recommended for an anterior lumbar interbody fusion at L5-S1, but the injured worker did not wish to undergo surgery.  After a period of vocational rehabilitation, the Claimant returned to work in a lighter physical capacity job.  The claim was heard for permanency, and the Commission found 25% permanent partial disability, with 20% related to the accidental injury.  Claimant filed an appeal and the claim was resolved through mediation.  Prior to the permanency hearing and appeal, the Claimant was involved in a subsequent work-related accident to the same part of the body.

 

  • Injury: Central extruded disc at L5-S1 with bilateral S1 nerve root compression.
  • Body Part: Lumbar Spine

  • Worker sustained a low back injury while moving heavy boxes. An MRI revealed a herniated disc at L5-S1.  A series of transforaminal injections did not bring relief and the injured worker was recommended for a L5-S1 transforaminal lumbar interbody fusion.  The Claimant did not wish to undergo the surgery.  Claimant did not return to work in order to assist a parent with a long-term health condition.

 

  • Injury: Mild degenerative joint disease and a low grade sprain of the ACL and mild to moderate chondromalacia of the patella, and a patellar subluxation

  • Worker felt a “pop” in the knees while scanning and stocking product. MRIs revealed mild degenerative joint disease and a low grade sprain of the ACL and mild to moderate chondromalacia of the patella, and a patellar subluxation.  The injured worker underwent conversative care and returned to work for a different employer.

 

 

  • Injury: Pneumothorax, ischiocavenosus disruption, grade II spleen injury, renal artery Grade V injury, partial tear of the right abdominal wall, partial tear of the iliac crest, transverse T12 fracture, transverse fractures at L1 through L4.  Minimally displaced various other fractures. Minimally displaced fracture of the ankle at the neck of the talus.  Non-displaced fracture to the medial malleolar facet of the talus.

  • Claimant was working as a tree cutter as part of the essential part of the job when the tree split and a limb weighing multiple hundreds of pounds struck him in the chest, back and other body parts. Injured worker was taken to Shock Trauma and was kept in patient for approximately seven days, at which point the worker was transferred to a rehabilitation hospital.  After a period of convalesce at home, the worker returned back to work.

 

  • Injury Multiple, but all injuries occurred during employment with Bethlehem Steel Corp.
  • Body Part: Occupational Hearing Loss and Tinnitus

  • Location: Baltimore County
  • Employer: Bethlehem Steel Corp.
  • Clients were employees of Bethlehem Steel Corp. at Sparrows Point. They were all diagnosed with occupational hearing loss.  Many also experienced tinnitus.  The Uninsured Employers’ Fund/Corvel denied medical benefits to the injured workers, including audiology testing, examinations, hearing aids and batteries.  After multiple hearings, the injured workers reached an agreement with the Uninsured Employers’ Fund to reinstate all medical benefits and to reimburse the injured worker for any out of pocket expenses.

 

  • Injury: Spiral Fracture of the distal fibula
  • Body Part: Ankle
  • Amount: $6,545.00
  • Claimant was stepping off of a truck when they sustained an ankle injury. An X-Ray revealed a minor distal fracture.  Claimant was placed in a boot for a period of time, followed by physical therapy and medications.  Claimant returned to the same job with the same employer.

 

  • Injury: Traumatic Injury to the hand

  • Claimant was attempting to detain someone when their hand was injured.

  • Claimant was injured when a piece of wood punctured their thumb, ultimately causing an infection. Claimant underwent surgery to drain the infection, followed by a course of antibiotics.  Claimant was also injured in an automobile accident when they were rear ended from behind. Claimant had a short course of physical therapy and was released back to full duty.  Claimant sustained an injury to the back while injured lifting heavy objects for work.  Claimant underwent an MRI, which revealed a disc protrusion, for which the claimant received treatment in the form of physical therapy, medications, before being released back to the same job with the same employer.

 

  • Injury: Musculoligamentous sprain of neck and back.

  • Claimant was injured while tripping over an object while at the workplace. An MRI revealed some degenerative changes at along with a disc protrusion. Claimant received physical therapy and medications before being released to the same job with the same employer.

  • Claimant was unloading luggage when they were struck in the back/hip area with a cart, sustaining a traumatic injury. MRI revealed a bulging disc in the back, and a potential minor tear of the labrum in the hip. Claimant underwent one injection, followed by physical therapy and treatment with medications, before being released to full-duty employment.

  • Claimant was at work when they fell off a ladder sustained a displace fracture of the foot. Claimant underwent surgery with hardware that was unsuccessful.  A second surgery was performed in the form of a midfoot fusion.  A third surgery was contemplated but declined.
