Claimant was initially evaluated and treated at an urgent care facility where a brace and pain medication was prescribed.  An MRI was performed and showed myxoid degeneration of the anterior and posterior horns of the meniscus.  Claimant then came under the care of an orthopedist and had a course of physical therapy.  Surgery was performed in the form of an arthroscopic partial medial meniscectomy.  This was followed by a course of post-operative physical therapy and Euflexxa injections.
