• Injury: Rotator cuff tear

• Claimant sustained a shoulder injury lifting a heavy couch. The Claimant had a cortisone injection followed by an MRI which showed a rotator cuff tear.   Surgery was performed in the form of an open rotator cuff repair.  Claimant underwent a course of post-operative physical therapy followed by work hardening but unfortunately was unable to return to preinjury employment.

 

• Injury: Cervical disc protrusion

• Claimant was injured at work when a box fell causing injury to the neck. An MRI was performed which revealed a protruding disk in the cervical spine. The injured workers underwent surgery in the form of a cervical discectomy and fusion. Additional treatment included a course of physical therapy and injections. The Claimant received a full duty discharge upon conclusion of treatment.

 

• Injury: Ankle fracture

• Claimant slipped and fell at work, sustaining a fractured ankle. The injured worker required surgery in the form of an open reduction with internal fixation of the medial malleolus. A second surgery was later performed to remove the hardware that was placed during the first procedure.  This was followed by a course of post-operative physical therapy.

• Claimant was lifting a couch with a co-worker and felt a pop in the shoulder. An MRI revealed a torn rotator cuff.  The Claimant had surgery performed to repair the tear.  This was followed by post-operative physical therapy.

• Claimant was involved in a motor vehicle accident during the course of employment. The Claimant came under the care of an orthopedist and pain management specialist. The initial treatment included physical therapy and injections.  Eventually, surgery was required in the form of an anterior cervical discectomy and fusion.

• Claimant fell at work and developed headaches and back pain. Claimant’s treatment included medication and physical therapy.

• Claimant was working as a lab technician doing repetitive tasks such as typing and uncapping tubes, when pain in the hand and wrist developed. The Claimant received an injection in the wrist.

 

• Injury: Rupture of ulnar collateral ligament of thumb

• Claimant was working as an electrician while using a power drill sustained injury to the thumb. An MRI revealed a high-grade partial tear of the ulnar collateral ligament.  Reconstruction surgery was performed and was the thumb was placed in a splint. Claimant underwent post-operative physical therapy and was released.
